The Faith Community Representative Program is designed to foster an on-going relationship between Four Seasons and area faith communities. Four Seasons’ goal is to meet the needs of our community by providing the very best in end-of-life care. This program promotes awareness of services provided by Four Seasons through trained volunteers who serve as a liaison with their faith community.
The Faith Community Representatives have received formal training and attend on-going educational classes to remain knowledgeable about Four Seasons and its services, which include hospice, palliative care, research, and Elizabeth House (inpatient facility). In addition, the volunteers arrange for speakers, written material, programs, etc. at the request of their communities.

- Nursing Homes
- Assisted Living Facilities
- Home Care Services
- Hospitals
- Elizabeth House, inpatient facility
- Bereavement Counselors to grieving family members
- Hospice Thrift Home Store
